SUBJECT: NOTICE OF COMPLETION FOR EISENHOWER PARK
RESTROOMS REMODEL PROJECT NO PRO901
SUMMARY OF REQUEST:
It is requested that the City Council adopt Resolution No. 6132 to formally accept
completion of the Eisenhower Park Restrooms Remodel Project No PRO901,
direct the City Clerk to file a "Notice of Completion" with the Orange County
Clerk- Recorder within ten (10) days from the date of acceptance, and to release
retention in the amount of $6,925.71 in 35 days.

BACKGROUND AND ANALYSIS:
On December 13, 2010, City Council awarded a contract to Unique Performance
Construction for the Eisenhower Park Restrooms Remodel Project No. PRO901
in the amount of $60,600.
The Eisenhower Park Restrooms Remodel Project involved replacement of the
existing entrance doors and frames, upgrading the ventilation system, new light
fixtures, wall painting, floor sealant, and other appurtenances.
Several change orders were encountered on the project; all were requested by
the City during construction. These include: adding door stops to keep the doors
open during the day, changing out the door to the plumbing corridor, modifying
the HVAC ducting during construction and bringing in temporary portable toilets
to handle the demand during the warmer weather while the pier restrooms were
out of service during construction.
The contractor was successful in meeting the City's expectations, including fast -
tracking the construction to have the restrooms open in time for the 5K/10K race
weekend. The project is now complete with the satisfaction of the City Engineer
and formal acceptance of the project is recommended.

This project was completed on time. The project was inspected and it meets all
standards and requirements within the project specifications. It is customary to
formally accept the project and file a notice of completion with the Orange County
Clerk.
ENVIRONMENTAL IMPACT:
This project has complied with all requirements of the California Environmental
Quality Act (CEQA).
FINANCIAL IMPACT:
The project is funded through the 2002 Resources Bond Act Block Grants. The
City received grant funds of $86,124 to cover the cost of design, construction and
inspection and there is no City match requirement. The final cost to the
contractor was $69,257.10 including all change orders. The final design and
inspection cost was $20,279.93. This brings the entire project cost to
$89,537.03. The $3,413.03 in additional expenses was paid out of the General
Fund Building Maintenance - Contract Services account 001- 052 - 44000.
RECOMMENDATION:
It is recommended that the City Council adopt Resolution No. 6132 to formally
accept completion of the Eisenhower Park Restrooms Remodel Project No
PRO901, direct the City Clerk to file a "Notice of Completion" with the Orange
County Clerk- Recorder within ten (10) days from the date of acceptance and to
release retention in the amount of $6,925.71 in 35 days.

A RESOLUTION OF THE SEAL BEACH CITY COUNCIL
DECLARING WORK TO BE COMPLETED PER CONTRACT
DOCUMENTS FOR THE EISENHOWER PARK RESTROOMS
REMODEL PROJECT NUMBER PRO901
WHEREAS, on December 13, 2010 the City of Seal Beach entered into a
contract with Unique Performance Construction; and
WHEREAS, the City Engineer has determined that the work has been
substantially completed in accordance with the contract documents.
NOW, THEREFORE, THE SEAL BEACH CITY COUNCIL DOES HEREBY
RESOLVE:
Section 1. That the work required to be performed by Unique Performance
Construction has been completed.
Section 2. That the total cost of amount to be paid to Unique Performance
Construction is $69,257.10 for the work performed.
Section 4. That the work is hereby accepted and approved.
Section 5. It is further ordered that a "Notice of Completion" be filed on behalf
of the City in the Office of the Orange County Recorder within 10
days of the date of this resolution.
