SUBJECT: NOTICE OF COMPLETION FOR 2011 ANNUAL
CONCRETE REPAIR PROGRAM PROJECT NO. ST1107
SUMMARY OF REQUEST:
It is requested that the City Council adopt Resolution No. 6163 to formally accept
completion of the 2011 Annual Concrete Repair Program Project No ST1107,
direct the City Clerk to file a "Notice of Completion" with the Orange County
Clerk- Recorder within ten (10) days from the date of acceptance, and to release
retention in the amount of $11,969.50 in 35 days.
BACKGROUND AND ANALYSIS:
On March 14, 2011, City Council awarded a contract to Golden State
Constructors, Inc. for the 2011 Annual Concrete Repair Program Project No.
ST1107 in the amount of $107,970.
The 2011 Annual Concrete Repair Program Project involved the repair of
sidewalks, curb & gutter, driveways and installation of new access ramps. The
access ramps are necessary to provide pedestrian access in accordance with the
American Disability Act (ADA).
In the attempt to rehabilitate all remaining asphalt patches, staff additionally
identified several locations throughout the City to be included for repair. The
majority of the asphalt patches have been eliminated with this project. The
remaining asphalt patch locations, primarily in Old Town and Bridgeport areas,
will be addressed with the upcoming 2012 Annual Concrete Repair Program
Project.
The contractor was successful in meeting the City's expectations, addressed
resident concerns that occurred during construction, and completed the
construction on time. The project was inspected and it meets all standards and
requirements within the project specifications.

The project is now complete with the satisfaction of the City Engineer and formal
acceptance of the project is recommended. It is customary to formally accept the
project and file a notice of completion with the Orange County Clerk.
ENVIRONMENTAL IMPACT:
This project has complied with all requirements of the California Environmental
Quality Act (CEQA).
FINANCIAL IMPACT:
The 2010/11 Adopted Budget, General Fund in the amount of $200,000 was
allocated for 2011 Annual Concrete Repair Program Project No. ST1107. On
March 14, 2011, a contract was awarded to Golden State Constructors, Inc. in
the amount of $107,970. During construction, staff identified additional locations
to be repaired. These locations had asphalt patches, which increased the
construction cost by $11,725.
The total cost of amount paid to Golden State Constructors, Inc. for construction
is $119,695 and $5,900 for inspection totaling to $125,595.
RECOMMENDATION:
It is recommended that the City Council adopt Resolution No. 6163 to formally
accept completion of the 2011 Annual Concrete Repair Program Project No.
ST1107, direct the City Clerk to file a "Notice of Completion" with the Orange
County Clerk- Recorder within ten (10) days from the date of acceptance, and to
release retention in the amount of $11,969.50 in 35 days.

A RESOLUTION OF THE SEAL BEACH CITY COUNCIL
DECLARING WORK TO BE COMPLETED PER CONTRACT
DOCUMENTS FOR THE 2011 ANNUAL CONCRETE REPAIR
PROGRAM PROJECT NO. ST1107
WHEREAS, on March 14, 2011 the City of Seal Beach entered into a contract
with Golden State Constructors, Inc.; and
WHEREAS, the City Engineer has determined that the work has been
substantially completed in accordance with the contract documents.
NOW, THEREFORE, THE SEAL BEACH CITY COUNCIL DOES HEREBY
RESOLVE.
Section 1. That the work required to be performed by Golden State
Constructors has been completed.
Section 2. That the total cost of amount to be paid to Golden State
Constructors, Inc. is $119,695 for the work performed.
Section 4. That the work is hereby accepted and approved.
Section 5. It is further ordered that a "Notice of Completion" be filed on behalf
of the City in the Office of the Orange County Recorder within 10
days of the date of this resolution.
