Crumby P.E., Assistant City Manager/Public Works QUBJECT: NOTICE OF COMPLETION FOR 2012 ANNUAL SLURRY SEAL PROJECT NO. STI 101 M IT, I , 71-,": -TESTS jjgguq�m It is requested that the City Council adopt Resolution No. 6919 accepting 2012 Annual Slurry Seal Project No. ST1101 by American Asphalt South, Inc. for $98,818.14 and direct the City Clerk to file a "Notice of Completion" with the Orange County Clerk-Recorder within ten (10) days from the date of acceptance and release retention in 35 days. On May 29, 2012, City Council awarded a contract to American Asphalt South, Inc. for the 2012 Annual Slurry Seal Project No. ST1 101 for $86,731.50. This project consisted of slurry sealing several streets in Old Town as identified in the Pavement Management Program Report. Application of slurry seal is typically done every 5-7 years, extending the effective life of asphalt concrete pavement by placing a protective seal over the existing pavement surface. The following streets were slurry sealed: • Electric Ave. from 6th St to Main St. • Seal Beach Blvd. from Electric Ave. to PCH • 11th St. through Neptune Ave. from Ocean Ave. to End • 10th Street Beach Parking Lot On July 23, 2012, the City Council accepted grant funds in the amount of $8,000.00 from the Run Seal Beach Committee designated to resurfacing basketball courts. A change order was generated to resurface and stripe the basketball courts at Edison Park and Almond Park. The project is now complete to the satisfaction of the City Engineer. The project was inspected and it meets all standards and requirements within the project Agenda Item E specifications. It is required to formally accept the project and file a notice of completion with the Orange County Clerk. ENVIRONMENTAL IMPACT: This project has complied with the California Environmental Quality Act. The City Attorney has review and approved as to form. FINANCIAL IMPACT: In the approved FY 2011-12 Budget $100,000 is budgeted for the 2012 Annual Slurry Seal Project No. ST1101. Below is a summary and breakdown of expenses for the project: Description Amount Construction $ 90,818.14 Change Orders* $ 8,000.00 Inspection $ 5,265.00 Total $ 104,083.14 *General Fund — Recreation Admin account number 001-070-40700 was used to pay the amount. The total amount paid to American Asphalt South, Inc. for construction/change orders is $98,818.14. NOW, THEREFORE, THE SEAL BEACH CITY COUNCIL DOES HEREBY RESOLVE: Section 1. That the work required to be performed by American Asphalt South, Inc. has been completed and accepted. Section 2. That the total cost of amount to be paid to American Asphalt South, Inc. is $98,818.14 for the work performed. Section 3. That a "Notice of Completion" be filed on behalf of the City in the Office of the Orange County Recorder within 10 days of the date of this resolution.
