SUBJECT: NOTICE OF COMPLETION FOR 2014 ANNUAL SLURRY
SEAL PROJECT NO. ST1•01
That the City Council adopt Resolution No. 6486:
1. Accepting the 2014 Annual Slurry Seal Project No. ST1401 by Roy Allan
Slurry Seal, Inc. in the amount of $37,834; and
2. Directing the City Clerk to file a "Notice of Completion" with the Orange
County Clerk-Recorder within ten (10) days from the date of acceptance
and to release retention in the amount of $1,891.70 in 35 days.
On April 28, 2014, the City Council awarded a contract to Roy Allan Slurry Seal,
Inc. for the 2014 Annual Slurry Seal Project No. ST1401 in the amount of
$40,184.
The 2014 Annual Slurry Seal Project No. ST1401 consisted of applying a slurry
seal to selected streets throughout the City as identified in the 2013-2014
Pavement Management Program Report. Slurry seal is typically applied every 5-
7 years and extends the effective life of asphalt concrete pavements. The streets
slurry sealed were as follows:
• Hazelnut Avenue from Heather St. to Rose St.
• Camelia Street from Almond Ave. to Banyan Ave.
The project is now complete to the satisfaction of the City Engineer. The project
was inspected and it meets all standards and requirements within the project
specifications. It is required to formally accept the project and file a notice of
completion with the Orange County Clerk.

LEGAL ANALYSIS:
No legal analysis is required for this item.
FINANCIAL IMPACT:
This project is budgeted in the approved FY 2013/14 Budget for $50,000. Design
plans were prepared in-house. Below is a summary and breakdown of expenses
for the project:

The total cost paid to Roy Allan Slurry Seal, Inc. for construction totals to
$37,834.
That the City Council adopt Resolution No. 6486:

A RESOLUTION OF THE SEAL BEACH CITY COUNCIL
ACCEPTING COMPLETION OF THE 2014 ANNUAL SLURRY
SEAL PROJECT NO. ST1401 AND DIRECTING THE FILING OF
A NOTICE OF COMPLETION FOR THE PROJECT
THE SEAL BEACH CITY COUNCIL DOES HEREBY RESOLVE:
Section 1. The City accepts the completion of the 2014 Annual Slurry Seal
Project No. ST1 404 (the "Project") by Roy Allan Slurry Seal, Inc. in the amount of
$37,834 for the work performed.
Section 2. The City Clerk is hereby directed to file a a "Notice of Completion"
for the Project with the Office of the Orange County Recorder within 10 days of
the date of this resolution. The retention in the amount of $1,891.70 shall be
released in 35 days.
