SUBJECT: NOTICE OF COMPLETION FOR 2014 ANNUAL
CONCRETE REPAIR PROGRAM PROJECT NO. ST1404
SUMMARY OF REQUEST:
That the City Council adopt Resolution No. 6501 :
1. Accepting the 2014 Annual Concrete Repair Program Project No. ST1404
by Golden State Constructors, Inc. in the amount of $55,455; and
2. Direct the City Clerk to file a "Notice of Completion" with the Orange
County Clerk-Recorder within ten (10) days from the date of acceptance
and to release retention in 35 days.
BACKGROUND AND ANALYSIS:
On April 28, 2014, the City Council awarded a contract to Golden State
Constructors, Inc. for the 2014 Annual Concrete Repair Program Project No.
ST1404 in the amount of $50,455.
The 2014 Annual Concrete Repair Program Project No. ST1404 consisted of
repairs identified within year two of the 2012 Update to the Citywide Concrete
Inspection Report, which involved repairing of sidewalks, driveways, and curb &
gutter citywide.
The project is now complete to the satisfaction of the Deputy Director of Public
Works/City Engineer. The project was inspected and it meets all standards and
requirements within the project specifications. It is required to formally accept the
project and file a notice of completion with the Orange County Clerk.
ENVIRONMENTAL IMPACT:
Pursuant to 14 Calif. Code of Regs. Section 15300, City staff determined that this
project is categorically exerript under the California Environmental Quality Act
(CEQA) Section 15301, Class 1 subsection (c), which includes the repair and

maintenance of existing public facilities that involve negligible or no expansion of
existing uses.
LEGAL ANALYSIS:
The City Attorney has reviewed the proposed resolution and approved as to
form.
FINANCIAL IMPACT:
This project is budgeted in the approved FY 2013/14 Budget for $60,500. Design
plans were prepared in-house. Below is a summary and breakdown of expenses
for the project:

The total cost paid to Golden State Constructors, Inc. for construction totals to
$55,455.
RECOMMENDATION:
That the City Council adopt Resolution No. 6501:
1. Accepting the 2014 Annual Concrete Repair Program Project No. ST1404
by Golden State Constructors, Inc. in the amount of $55,455; and
2. Direct the City Clerk to file a "Notice of Completion" with -the Orange
County Clerk-Recorder within ten (10) days from the date of acceptance
and to release retention in 35 days.

ACCEPTING COMPLETION OF THE 2014 ANNUAL CONCRETE
REPAIR PROGRAM PROJECT NO, ST1404 AND DIRECTING
THE FILING OF A NOTICE OF COMPLETION FOR THE
PROJECT
THE SEAL BEACH CITY COUNCIL DOES HEREBY RESOLVE:
SECTION 1, The City accepts the completion of the 2014 Annual Concrete
Repair Program Project No. ST1404 (the "Project") substantially completed by
Golden State Constructors, Inc. in accordance with the contract documents,
dated on April 28, 2014 for the total cost of$55,455.
SECTION 2, The City Clerk is hereby directed to file a"Notice of Completion"for
the Project with the Office of the Orange County Recorder within 10 days of the
date of this Resolution. The retention shall be released in 35 days,
PASSED, APPROVED and ADOPTED by the Sea[ Beach City Council at a
regular meeting held on the 8th day of September 2014 by the following vote:
