SUBJECT: NOTICE OF COMPLETION FOR 2015 ANNUAL SLURRY
SEAL PROJECT NO. ST1501
SUMMARY OF REQUEST:
That the City Council:
1. Adopt Resolution No. 6556 accepting the 2015 Annual Slurry Seal Project
No. ST1501 by Doug Martin Contracting Co., Inc. in the amount of
$56,542.96; and
2. Direct the City Clerk to file a "Notice of Completion" with the Orange
County Clerk-Recorder within ten (10) days from the date of acceptance
and to release retention in 35 days.
BACKGROUND AND ANALYSIS:
On March 9, 2015, the City Council awarded a contract to Doug Martin
Contracting Co., Inc. for the 2015 Annual Slurry Seal Project No. ST1501 in the
amount of$62,709.00.
Streets within the Annual Slurry Seal Program are identified within the Pavement
Management Plan (PMP). Due to an estimated shortfall in funding, the 2015
Annual Slurry Seal Project No. ST1501 did not deliver all of the streets identified
within the PMP. The streets slurry sealed within the 2015 Slurry Seal Program
ST1501 are as follows:
• Bluebell Street from Birchwood Avenue to Almond Avenue
• Almond Avenue from Bluebell Street to Jasmin Circle
The pending streets for 2015 will be included with the FY 2015/16 Annual Slurry
Seal Project and any unused funds will be carried over.
The project is now complete to the satisfaction of the Deputy Director of Public
Works/City Engineer. The project was inspected and it meets all standards and

requirements within the project specifications. It is required to formally accept
the project and file a notice of completion with the Orange County Clerk.
ENVIRONMENTAL IMPACT:
There is no environmental impact related to this item.
LEGAL ANALYSIS:
No legal analysis is required for this item.
FINANCIAL IMPACT:
This project is budgeted in the approved FY 2014/15 Budget for $100,000.
Design plans were prepared in-house. Below is a summary and breakdown of
expenses for the project:

The total cost paid to Doug Martin Contracting Co., Inc. for construction is
$56,542.96.
RECOMMENDATION:
That the City Council:
1. Adopt Resolution No. 6556 accepting the 2015 Annual Slurry Seal Project
No. ST1501 by Doug Martin Contracting Co., Inc. in the amount of
$56,542.96; and
2. Direct the City Clerk to file a "Notice of Completion" with the Orange
County Clerk-Recorder within ten (10) days from the date of acceptance
and to release retention in 35 days.

A RESOLUTION OF THE SEAL BEACH CITY COUNCIL
DECLARING WORK TO BE COMPLETED PER CONTRACT
DOCUMENTS FOR 2015 ANNUAL SLURRY SEAL PROJECT
NO. ST1501
WHEREAS, on March 9, 2015 the City of Seal Beach entered into a contract
with Doug Martin Contracting Co., Inc. in the amount of$62,709.00; and
WHEREAS, the Deputy Director of Public Works/City Engineer has determined
that the work has been substantially completed in accordance with the contract
documents.
NOW, THEREFORE, THE SEAL BEACH CITY COUNCIL DOES HEREBY
RESOLVE:
Section 1, That the work required to be performed by Doug Martin Contracting
Co., Inc. has been completed and accepted.
Section 2. That the total cost of amount to be paid to Doug Martin Contracting
Co., Inc. is$56,542.96 for the work performed.
Section 3. That a "Notice of Completion" be filed on behalf of the City in the
Office of the Orange County Recorder within 10 days of the date of
this resolution and release retention in 35 days.
