Water Street Orange, California 92866 Subject: Extension of Agreements for Emergency Medical Transportation Dear Chief Howlind: Medix Ambulance Service respectfully requests the Orange County Fire Authority to extend our agreements to provide ambulance service for the additional three Permitted years in the following ASA's: #10 (Irvine); #13 (La Palma); #14 (Los Alamitos); #21 (Seal Beach); #22 (Stanton); and #24 (Villa Park). As you know, our current agreements expire June 30, 2001. The extensions, if approved, would make the new expiration dates June 30, 2004. It is anticipated that by that time a new RFP process will have been conducted. Medix considers it a privilege to serve the OCFA, and will continue to meet or exceed the performance requirements that have provided the merit for this request.
